Hi, i want to take my dog back home, already did all the shots and have a dog's passport with marks, but no info about myself, where i can do it? Because the vet who did the shot said i gotta go to a some special place to fill in this information, if any one knows that place please let me know! Thank You

The vet should have told you where to go. There are many vets but prob only one who is licensed to stamp this form, but it is just another vet office. I would suggest you call other vets and ask, someone will know. Your time is crucial as you probably need the forms filled out no more than 10 days before leaving or something like that.

Try contacting the airline, they should be able to give you the name of the Bureau that deals. Even if they can't tell you exactly where it is, knowing the name will help narrow things down.
